The surgical management of venous disease is filled with dichotomies. Procedures span the spectrum from the most mundane saphenous vein stripping (one of the first procedures I did as an intern) to sophisticated central venous reconstructions. Venous surgery tends not to be glamorous, playing second fiddle to arterial reconstruction but frequently is technically more demanding and very dependant on meticulous technique. Diagnosis may be straightforward as in identifying lower extremity varicosities or extremely challenging as in confirming a true thoracic outlet syndrome. Though responsible for a great deal of morbidity in the population as a whole, it receives less attention than it deserves from caregivers. The patient who presents with DVT now frequently is sent home immediately on injectable anticoagulants “per protocol,” deceptively minimizing the long-term sequelae of chronic venous stasis disease. Venous disease is the Rodney Dangerfield of surgery, getting little respect. This is a mistake as Dr. Illig and his colleagues capably illustrate in this issue of Operative Techniques in General Surgery. They show the numerous surgical options available to treat various venous disorders and the evolving nature of those treatments. This broad reaching issue spans newer treatments for the lowly varicosity all the way to the complexities of vena cava and subclavian vein reconstruction. Just as tackling venous disease takes a special kind of surgeon with a special commitment to succeed, so did Dr. Illig take on the challenging task of organizing this issue, something that a less motivated individual might shy away from. He has recruited a truly elite group of surgeons to create this unique issue on this often-minimized surgical topic. I thank him and his contributors sincerely for a valuable compendium that brings to the forefront the surgical options available to the patient with venous disease.

In 1981, the Surgical Clinics of North America published an issue entitled “Biliary Tract Disease” . This predated any of our attempts at laparoscopic biliary surgery and was state-of-the-art. In 1994, the Clinics published “Biliary Tract Injuries Revisited,” straddling the era of “all open” biliary surgery and emerging laparoscopic surgery .This issue of the Surgical Clinics of North America will try to add on to the traditions of biliary surgery set forth in previous issues. We have learned much about the mechanisms and prevention of laparoscopic bile duct injuries in the last 20 years, and a review of pertinent anatomy and operative techniques will be presented. Diagnosis, palliation, and surgical management of injuries and pathology of the biliary tract have advanced logarithmically. Radiologic techniques are emerging that provide three-dimensional reconstructions of complex biliary anatomy. Endoscopic imaging and treatment options are evolving as rapidly as laparoscopic instrumentation. All of these new technologies are leading to safer, more effective management of proximal and distal biliary malignancy, biliary reconstruction, and preoperative planning for all biliary surgery. We hope that this issue will prove a useful tool both for surgeons just beginning their practice and more senior surgeons whose careers (sometimes anxiously) have bridged the eras of open and laparoscopic surgery.

1.For all courses, where MBBS/BDS is an eligible requirement, the candidate who have made more than one attempt (i.e. have more than one failure, compartment or reappear) during their MBBS / BDS career, are not eligible. For DM/M.Ch., courses, the candidates who have made mroe than one attempt in MBBS/MD/MS career are not eligible. However, those belonging to Sch. Caste/Tribes with upto two attempts in their MBBS/BDS career will be eligible for MD/MS, and MDS, House Job in Oral Health Sciences.
2.The candidates completing internship after 31.12.2008 are not eligible for course No. 1, 2 and 3.
3.The number of seats wherever indicated are provisional and may increase/decrease without any prior notice.
4.The course at Sr. No. 6 (1-13) are only for Sponsored / Deputed Candidates.
5.A candidate applying for more than one subject/course except category – 1 & 2 is required to submit separate application complete in all respect for reach subject / course.
6.The candidates may apply in the form downloaded (for Sr. No. 1 to 4) at http://pgimer.nic.in together with requisite fee by Demand Draft in the name of Director, PGI. The prescribed application forms alongwith prospectuses (Brochure of Information) are also available from the office of the undersigned either personally on payment of Rs.500/- for GENERAL/OBC/OPH category and Rs.400/- for SC/ST at the counter (Kairon Block, Room No. 307) from 10.30 AM to 12.30 PM. And from 2.30 PM to 3.30 PM on all working days except Saturdays (On Saturdays, forms will be available from 10.30 AM to 12.00 Noon) or by post for which the request must be accompanied with a self addressed thick envelope of size 24×15 cm bearing postage stamps of Rs.30/- and Bank Draft preferable form any SBI branch payable at SBI Medical Institute Branch (Code No. 1524), Chandigarh for GENERAL/OBC/OPH Rs.500/- and for SC/ST Rs.400/- (indicating category i.e. General or SC/ST) drawn in faovur of the Director, PGI, Chandigarh. However, prospectus of DM/M.Ch. will be available for Rs. 500/- only as there is no reservation for the same.
